Mental Health Social Worker plans and consults patients’ discharge with patients, families and the health care team. Mental Health Social Worker prepares the patient and family to manage patient hospitalization, illness, diagnosis, treatment, and/or life situation, including emotional, mental, and substance abuse disorders.
